Shared Care Plan Coordinator

1.0 FTE Dunedin or Invercargill

 

WellSouth has the aim of improving health outcomes for people in the Southern district. Shared Care planning is now being instigated across the Southern Health System.

 

WellSouth is looking to employ a Coordinator to support the implementation of shared care planning. The role will provide leadership and coordinationand ensure the ongoing system improvements and processes relating to electronic use of personalized care plans, acute plans and advance care plans.

 

The successful applicant will:

  • Be able to forge strong relationships both internally with in WellSouth and externally within the General Practice, NGO Secondary Care and Community sector
  • have previous experience in teaching and mentoring with the ability to deliver clinical education and training programmes
  • have the ability to develop, redesign and improve existing  systems and process to ensure they at  fit for purpose and assist in delivery of good patient outcomes
  • be a registered health professional
