Long-term conditions nurses are involved in training GP practice staff so they can deliver the best care for you. They provide advice and support to GP practice staff. For example, if you work a variable shift and you are starting insulin, the Long Term Conditions Nurse can advise the practice on how best to manage your insulin alongside your shift work.
The Long Term Conditions team delivers DESMOND and Walking Away (from Diabetes) as group courses to help improve the health outcome of people who are at risk of or have Type 2 diabetes.
