Long-term Conditions
Our clinical team (nurses, pharmacists, dietitians) support general practice teams care for people with long-term conditions like diabetes, heart disease, gout or COPD.
The Long-term Conditions team are involved in training practice staff so they can deliver the best care for you. They provide advice and support to practice staff. The Long-term Conditions team delivers education on pre-diabetes and diabetes as group courses to the public to help improve the health outcome of people who are at risk of developing or have Type 2 diabetes.
The Primary Care Nurses work alongside General Practice, actively teaching (individual, small/large groups), facilitating and promoting enhanced management of long-term conditions across the Southern region. They support general practice teams to provide nursing services to clients via a variety of community bases that promote health, prevent illness and disease and provide support and treatment with a particular emphasis on diabetes, cardiovascular disease, respiratory illness and gout.
