Health Care Home Continuous Improvement Toolkit
The Health Care Home (HCH) model of care provides a framework for true patient centred care. It is a nationwide general practice model that supports primary care to deliver a better patient experience, improved quality of care, benefits for clinicians and greater practice efficiency. HCH introduces change to improve access for patients and to keep general practice sustainable, through a process of continuous improvement.
The continuous improvement toolkit is a series of online resources which help general practice to implement some of the core components of Health Care Home (HCH).
Each core component is broken down into sections including the benefits to the practice; the benefits to the patient; who does what within the practice; an implementation timeline; and resources & helpful tips.
