Outreach Nursing Services
WellSouth Nursing Outreach team provides support to patients, whānau and general practices across the Southern region. Their team's goal is to support the health of people in Southern by helping them to access primary care services, with a particular focus on Māori, Pacific and high needs populations.
Your GP surgery may refer you to the WellSouth Outreach team if you are finding it hard to keep appointments for health screening e.g. smears, diabetes or heart health checks.
Annual diabetes reviews and accessing vital screening programmes, such as cervical cancer and breast cancer screening programmes, as well as the National Bowel Cancer Screening Programme, are among the programmes the Outreach team supports.
Staffed by nurses in Dunedin, Invercargill and in Central Otago, the Outreach team accepts referrals from general practices to help them get patients to re-engage and access the health care they are eligible for.
The Team does follow-up calls by phone and in person to patients referred from practices.
Led by Clinical Services Manager Sharron Feist in Dunedin, Outreach staff include Racheal Maheno and Amanda Dodd in Invercargill, Brittany Butler and Sherlie Barnett and Karin Little in Dunedin and Rachael Hall in Central Otago.
