Notification of Problems by General Practice under Clause B.26 of the Primary Health Services Agreement
Please email us on PracticeNetworkTeam@wellsouth.org.nz to report any of the following issues that may put at risk the ability to deliver services and its contractual obligations to Te Whatu Ora (Te Waipounamu - Southern).
Examples to report

A patient complaint that meets the requirements of the Health and Disability Commission

Adverse events as per the HQSC National Adverse Events Reporting Policy

Any issue that is referred to a regulated Professional Body for any employed or contracted individual within the general practice

Any serious health event that may impact on WellSouth’s ability to meet its contractual obligations to Te Whatu Ora (Te Waipounamu - Southern) under the Primary Health Services Agreement

Financial stress at practice or financial irregularities relating to WellSouth programmes

Staff availability (such as illness, sudden resignation, etc)

Anything that has the potential to see a practice close at short notice, either permanently or temporarily
Anything that may impact on the provision of after-hours services

Any privacy breaches
