Notification of Problems by General Practice under Clause B.26 of the Primary Health Services Agreement

Please email us on PracticeNetworkTeam@wellsouth.org.nz to report any of the following issues that may put at risk the ability to deliver services and its contractual obligations to Te Whatu Ora (Te Waipounamu - Southern).

Examples to report
  • A patient complaint that meets the requirements of the Health and Disability Commission

  • Adverse events as per the HQSC National Adverse Events Reporting Policy

  • Any issue that is referred to a regulated Professional Body for any employed or contracted individual within the general practice

  • Any serious health event that may impact on WellSouth’s ability to meet its contractual obligations to Te Whatu Ora (Te Waipounamu - Southern) under the Primary Health Services Agreement

  • Financial stress at practice or financial irregularities relating to WellSouth programmes

  • Staff availability (such as illness, sudden resignation, etc)

  • Anything that has the potential to see a practice close at short notice, either permanently or temporarily

    Anything that may impact on the provision of after-hours services

  • Any privacy breaches
